"An American Werewolf in London" - the dream sequences. All of them are good but the two stand out ones, Nurse Price tending David in a hospital bed in the woods when he pulls that face and of course the dream where he's back home with his family and the doorbell rings... culminating in the best jump scene ever (well second to Carrie's "hand from the grave scene"
Classic scare moments.
There's also one from a Zombie flick who's name I can't recall. The scene is near the beginning where two guys are drinking Buds in the cockpit of a light aircraft and in the far window you see this face slowly appear, glance at the two oblivious blokes then disappear. Frightened the hell out of me as a kid. Never seen the film again and like I said don't even know what it's called.

The whole of "Requiem for a Dream"

The Squid Scene in "OldBoy", The tongue scene and ending in the same.

Another Vote for David Warner + Glass in Omen
